To the Board of the International Spine Society,

I have carefully reviewed the Charter of the International Spine Society and hereby declare my intent to become a member of your esteemed organization. I confirm the accuracy of the information provided above and affirm that I have no criminal record or legal restrictions that would prevent my membership. I undertake to notify the society officials of any changes to my residence address or contact information within three (3) days, to fulfill any duties assigned by the Board of Directors, and to actively work in line with the objectives of the Society.

Respectfully, I submit this request for your consideration and kindly ask for the approval of my membership.
